Three years ago, creatine was still a locker-room supplement β something you scooped into a shaker after leg day and forgot about until the next one. Now it's turning up in conversations about perimenopause, cognitive ageing and bone health, championed by physicians who've never set foot in a CrossFit box. That's a big shift for a compound that costs about ten pence a gram. So what does the actual research say, once you strip away the enthusiasm?
From gym bag to bathroom cabinet: why creatine got rebranded
The shift happened for a reason. Most of the original creatine research was done on young men lifting weights, which meant an entire population β women, particularly those past 40 β were left out of the picture for decades. Newer trials have started closing that gap, and what they've found isn't uniformly flattering. Some of it holds up well. Some of it doesn't. It's worth knowing which is which before you spend money on it.
What creatine is actually doing in your body
Creatine isn't a stimulant, a hormone, or anything exotic. Your liver and kidneys make a small amount of it daily from amino acids, and you top the rest up through red meat and fish β though not much, since a typical diet supplies roughly 1g a day. It's stored mostly in skeletal muscle, where it helps regenerate ATP, the molecule your cells burn for quick bursts of energy. The muscle evidence, and the one number worth remembering
On strength, the research is about as settled as supplement science gets. Creatine increases physical performance in successive bursts of short-term, high-intensity exercise β an effect established enough that it's one of the few creatine claims formally authorised on the EU health claims register, at a daily intake of 3g.[1] That's not a figure picked out of the air, either β it's exactly the dose our own tablets are built around.

There's a second authorised claim that matters more if you're over 55: taken alongside regular resistance training, daily creatine has been shown to enhance the effect of that training on muscle strength specifically in older adults.[2] That's a meaningfully narrower claim than "creatine makes you stronger" β it only holds alongside the resistance training. The tablet isn't doing the lifting for you.
Brain health: what the memory trials actually found
This is the part most of the recent headlines are about, and the honest answer is promising, but early. A 2023 systematic review and meta-analysis in Nutrition Reviews pooled ten randomised controlled trials and found creatine supplementation improved measures of memory compared with placebo β with the effect notably stronger in adults over 66 than in their younger counterparts.[3]

A separate 2024 meta-analysis in Frontiers in Nutrition reached a similar conclusion for broader cognitive function, though the authors were careful to note that study designs, doses and durations varied enough that larger, more consistent trials are still needed before this becomes settled science.[4] Read that as a real signal, not yet a guarantee.
Bone density: where the hype has outrun the evidence
Here's where it's worth being honest rather than convenient. The most rigorous test of creatine and bone health to date is a two-year, placebo-controlled trial in 237 postmenopausal women, who took 3g of creatine daily alongside a structured resistance and walking programme. At the end of two years, creatine made no measurable difference to bone mineral density at the femoral neck, hip or spine compared with placebo.[5] It did produce some modest, more technical improvements in bone geometry β the shape and load-bearing structure of bone, rather than its density β but that's a narrower result than "creatine builds bone." If bone health is your main reason for taking it, weight-bearing exercise, calcium and vitamin D are still doing most of the actual work.
How much to actually take, and what's worth checking on the label
The dose that shows up across nearly every trial above β muscle, brain and bone alike β is 3g a day, taken consistently. There's no meaningful benefit to cycling on and off, and no real need to "load" with higher doses first unless you're chasing faster saturation ahead of a specific event. Creatine monohydrate, the plain, unbuffered form used in essentially all of the research cited here, remains what most sports scientists still reach for. Newer forms like HCL or buffered creatine are marketed on absorption claims the evidence doesn't yet back up.

Long before anyone ran a randomised controlled trial, someone in a kitchen somewhere was grating fresh ginger into hot water because their stomach felt wrong. That's roughly three thousand years of tradition to live up to. The question worth asking now isn't whether ginger has a reputation β it clearly does β but whether the trial evidence actually backs it, and for which kind of queasy stomach specifically.
What "ginger for nausea" is actually built on
The active compounds are gingerols and shogaols β the same molecules responsible for ginger's heat. Dry or cook the root and gingerols convert to shogaols, which is part of why fresh ginger tea and powdered ginger capsules don't behave identically in the body. Most of the clinical research uses standardised powder, not a mug of tea, which matters when you're comparing a study's dose to what's actually in your cupboard.
Pregnancy nausea: the best-studied use case
This is where the evidence is strongest, and also where you should be most careful about self-directing. NHS guidance notes there's some evidence ginger may help with nausea and vomiting in early pregnancy, alongside the standard advice to check with a pharmacist before taking any ginger supplement while pregnant.[1] A systematic review and meta-analysis pooling multiple trials found ginger reduced nausea scores compared with placebo, though the effect on actual vomiting episodes was less clear-cut, and several of the underlying trials carried a real risk of bias.[2]
If you're pregnant, this is a talk-to-your-midwife-or-GP conversation, not a self-medicate one β particularly given ginger's mild blood-thinning properties, which we'll come back to.
Motion sickness, post-op nausea and chemo β thinner evidence
Outside pregnancy, the picture gets patchier. Trials on motion sickness and nausea after surgery generally point in ginger's favour, but the studies are smaller and less consistent than the pregnancy literature. For chemotherapy-related nausea, results across trials have been mixed enough that it isn't something oncology teams recommend as a substitute for prescribed anti-sickness medication β at best, a few people use it alongside, with their team's knowledge.

Ginger and digestion: what "helps support digestion" actually means
Ginger carries a permitted botanical claim in the UK and EU β "helps to support digestion" β used at a daily intake of 0.5 to 2g of root ginger.[3] Worth knowing: botanical claims like this one sit in a different regulatory category to vitamin and mineral claims. They're permitted under transitional "on hold" arrangements while the EU's wider review of botanical evidence remains unfinished, rather than fully re-evaluated the way a claim on, say, vitamin D would be. That doesn't mean the claim is baseless β the traditional-use case is genuinely substantial β but it's a different kind of evidence to a modern EFSA-assessed trial.

Who should be cautious

Ashwagandha has become the ingredient everyone name-drops the moment stress comes up β in gym forums, in group chats, on wellness TikTok, usually paired with some version of the claim that it βlowers cortisol.β That much isnβt invented. But the story behind it is narrower than the headlines suggest, and if you like knowing what a study actually found before you start taking something daily, this is worth five minutes.
What Cortisol Does β and Why Ashwagandha Gets Linked to It
Cortisol is your main stress hormone, released by your adrenal glands whenever your brain flags something as urgent β a deadline, an argument, actual danger. In short bursts, itβs useful: it sharpens focus and mobilises energy. The problem is chronic elevation, cortisol that stays high because life stays demanding, which tracks with poor sleep, low mood, and wider strain on the body over time.
Ashwagandha (Withania somnifera) is classed as an adaptogen β a category of plants used in Ayurvedic medicine for centuries on the idea that they help the body regulate its stress response rather than sedate it outright. A 2025 systematic review and meta-analysis in BJPsych Open pooled the available randomised controlled trials and found a significant effect of ashwagandha on cortisol levels compared with placebo, alongside reductions in perceived stress and anxiety scores.[1] Significant here means the effect is real and unlikely to be down to chance. It doesnβt mean dramatic, and it doesnβt mean identical for everyone who takes it.

The Study Behind the Number Youβve Probably Seen
Most of the specific percentages floating around online trace back to one trial: a 2012 double-blind, placebo-controlled study of 64 adults with a history of chronic stress, published in the Indian Journal of Psychological Medicine. Participants took 300mg of a high-concentration, full-spectrum ashwagandha root extract twice a day for 60 days. By the end, the treatment groupβs serum cortisol had dropped 27.9% relative to placebo, alongside improvements across several stress and anxiety scales.[2]
That extract is now sold under the brand name KSM-66, one of the most-studied standardised ashwagandha extracts on the market. Worth being precise about what this does and doesnβt prove: it was one trial, one dose, one specific extract, in adults who were already chronically stressed. Itβs not a guarantee of what happens with a different formulation, a different dose, or someone who isnβt especially stressed to start with.

The UK Safety Review Most Ashwagandha Content Skips
Hereβs the part most ashwagandha content leaves out. In 2024, the UK Food Standards Agency opened a call for evidence on ashwagandha specifically, citing βpotential risks to health from consuming ashwagandha in food supplementsβ β flagging case reports linking it to effects on thyroid hormone levels and, less commonly, liver toxicity.[3] The FSAβs own toxicology committee described the liver cases as consistent with an idiosyncratic reaction: rare, and not predictable from dose alone. Denmark restricted ashwagandha in food supplements back in 2020 on similar grounds, and the UK still hasnβt settled on formal usage limits β the review is genuinely ongoing.
Itβs also worth being straight that thereβs no EFSA-authorised health claim for ashwagandha to point to here. Taking It Sensibly
Give it time β most trials run 8 to 12 weeks before measuring results, so four days wonβt tell you much.
Match the studied dose range β most positive trials sit between 250mg and 600mg a day of standardised extract, not a rough guess at a powder serving.
Take it daily, not as a rescue remedy β the effect builds through consistent use rather than showing up after one dose.
Check with your GP first if thyroid medication, pregnancy, or liver health are part of your picture.
Ashwagandhaβs popularity has outpaced the caution around it, which tends to happen with anything that goes from ancient remedy to Instagram shorthand in a few years. The cortisol research is real and worth taking seriously. Itβs also tied to specific extracts, specific doses, and people who were properly stressed to begin with β not a blanket promise. Read the label, give it the weeks it needs, and donβt skip the GP conversation if your history calls for it. Thatβs the standard worth holding it to.

What ashwagandha is actually being asked to do
Ashwagandha (Withania somnifera) is a root used in Ayurvedic medicine for centuries, generally as a tonic for resilience under strain β what modern researchers call an βadaptogen.β The narrower claim being tested in labs today is whether it lowers cortisol, the hormone your adrenal glands release when your body registers a threat, whether that threat is a work deadline or an actual emergency. Chronically elevated cortisol is linked to poor sleep and low mood, among other things, so a supplement that turns the dial down is an obvious thing to want to work.
The cortisol evidence, specifically
The most-cited single trial here is a 2012 study out of Asha Hospital in Hyderabad: 64 adults with self-reported chronic stress took either 300mg of KSM-66 ashwagandha extract twice a day, or a placebo, for 60 days.[1] The ashwagandha group's serum cortisol dropped 27.9%, against a 7.9% drop in the placebo group β a meaningful gap for a two-month trial.

That's one trial, though, and supplement research doesn't always replicate outside the original lab. The stronger evidence is a 2025 systematic review pooling eight randomised controlled trials across 488 adults, which found a statistically significant average cortisol reduction compared with placebo.[2] Across different doses and extract types, cortisol genuinely does come down.
Why βlower cortisolβ isn't the same as βfeel less stressedβ
Here's the part most ashwagandha content skips. Reported stress, in the same participants, largely didn't move.
Maybe eight weeks isn't long enough for a hormonal shift to show up in how someone feels day to day. Maybe cortisol just isn't the right lever for subjective stress in the first place. Either read is honest. Neither one is the confident βashwagandha crushes stressβ framing you'll see on most supplement sites.
What UK regulators are actually looking at right now
This is the part worth knowing before you buy anything. The UK's Food Standards Agency ran a formal call for evidence on ashwagandha's safety in 2024 and is still working through what it means for future guidance, after risk assessments flagged possible effects on thyroid hormones, liver function and blood sugar at some doses.[3] Denmark has already restricted ashwagandha in food supplements over similar concerns, and other EU food safety bodies are watching closely.
None of this means ashwagandha is dangerous for most healthy adults at typical doses β the concerns centre on specific groups, not a blanket problem.[4] But it does mean skipping it in pregnancy or while breastfeeding, and talking to a doctor first if you have a thyroid condition, liver disease, or take thyroid, blood sugar or blood pressure medication. It's also worth knowing that ashwagandha has no authorised EFSA health claim, in either direction β inconsistent extracts and unclear mechanisms have kept it off the approved list for years.[5] That's not the same as βunsafe.β It just means nobody gets to print a guaranteed-benefit claim on the box, including us.
If you're going to take it, what actually matters
Assuming none of the above rules you out, the studies that showed results used specific things: a standardised KSM-66 extract, roughly 600mg a day, taken consistently for eight weeks or longer.[1] Loose ashwagandha powder from an unlabelled source, taken occasionally, isn't the same product the trials tested.
The extract type β KSM-66 and Sensoril are the two most-studied standardised extracts; generic βashwagandha root powderβ with no extraction ratio listed is a different, less-tested product.
The dose β most trial doses land around 600mg a day; considerably less than that is unlikely to match the results in the studies.
Absorption support β black pepper extract (piperine) is commonly added because it measurably improves uptake of the active compounds.
Third-party testing β a UK-manufactured product tested by an independent lab is a reasonable minimum bar for a herbal extract under this much regulatory scrutiny.

The evidence for ashwagandha and cortisol is genuinely better than most adaptogen claims you'll come across. It's just narrower than the marketing suggests. Cortisol comes down, trial after trial. Whether that translates into feeling calmer is still an open question, and whether it's right for you depends more on your health history than most supplements on the shelf.
